We Receive From You |
- We do not currently collect personal information
from Web site users or their computers ("Users").
DRHS may, in the future collect information
through the use of "cookies" on the
browser of your computer and/or through request
of personal information such as contact information,
health topic preferences, and so forth ("User
Information").
- Users may choose to create, develop and enter
information and materials onto the Web site which
may be transmitted and/or stored on the Web site
servers, and which, for purposes hereof, are
called Submissions . Submissions may include
information maintained by the User in the course
of providing health services to a patient concerning
the patient and services provided.

of Cookies |
- DRHS does not currently, but may in future,
place a "cookie" on the browser of
your computer. A cookie is a piece of data stored
on your hard drive containing information about
you. Usage of a cookie is in no way linked to
any personally identifiable information while
on our site. Once you close your browser, the
cookie simply terminates. A choice by DRHS to
use "cookies", would be to enhance
the performance of its Web site, to allow customized
features of the Web site to perform properly
for each User, to track pages which are being
hit and determine by whom, and to store demographic
information for the User to establish a setup/address
book. If DRHS employs cookies, you must allow
the cookie to be placed on your computer as
a condition of using the Web site.

of Information We Receive From You |
- Except as set forth in this section, we will
not disclose to any unaffiliated company any
of the User Information. Any information provided
to entities affiliated with us will be treated
in accordance with this Privacy Statement. We
will consider, and may release, User Information
to third parties to comply with valid legal
requirements such as a law, regulation, search
warrant, subpoena or court order. We will also
provide User Information to our vendors and
suppliers when it is necessary for them to provide
us with products and services related to better
operation and maintenance of our Web site (our
"Vendors"). We will request that each
Vendor not further use or disclose User Information
for any purpose other than providing us or you
with products or services. Of course, we cannot
guarantee their compliance with our requests.
- We use e-mail addresses to analyze trends,
administer our Web site, and track the movement
of our Users. We also use e-mail address to
gather broad demographic information that is
aggregated for our internal use, and we may
provide this information to third parties ("Aggregate
Information"). For example, we might inform
third parties regarding the number of Users
of our Web site and the activities they conduct

- DRHS will never disclose or share any Submissions
maintained on or exchanged through the Web site.
- DRHS does not compile, buy, sell, rent or

A PATIENT CONCERNING THE PATIENT AND SERVICES
PROVIDED. USERS SHALL ONLY DO THIS WHERE NECESSARY
IN CONNECTION WITH THE CARE OF THE PATIENT AND
IN THE NORMAL COURSE OF CLIENT'S BUSINESS IN
ACCORDANCE WITH ACCEPTED STANDARDS OF PRACTICE
WITHIN THE HEALTH SERVICES SETTING. BY CHOOSING
TO USE THIS WEB SITE TO TRANSMIT SUCH INFORMATION,
YOU DO SO AT YOUR OWN RISK AND THAT OF THE CLIENT.
IF YOU CHOOSE TO USE THIS WEB SITE TO TRANSMIT
SUCH INFORMATION, YOU MUST OBTAIN PRIOR WRITTEN
AUTHORIZATION OF THE PATIENT TO DO SO ("PATIENT
AUTHORIZATION"). YOU AGREE TO ALLOW DRHS
TO REVIEW, UPON DEMAND, ANY AND ALL APPLICABLE
PATIENT AUTHORIZATIONS. PURSUANT TO THIS WEB
SITE'S TERMS AND CONDITIONS. YOU AND THE CLIENT
HEREBY INDEMNIFY, DEFEND AND HOLD HARMLESS DRHS,
ITS OFFICERS, DIRECTORS, MEMBERS, EMPLOYEES
AND AGENTS FROM AND AGAINST ANY AND ALL CLAIMS,
COSTS, LOSSES, FINES, PENALTIES AND LIABILITY
(INCLUDING REASONABLE ATTORNEYS' FEES THROUGH
FINAL APPEAL) FOR ANY FAILURE TO OBTAIN A PATIENT
AUTHORIZATION.
- We may from time to time seek to have our
site certified by independent certification
organizations. Such certifications may require
an audit of our Web site, including your User
Information, Submissions and the Aggregate Information.
We will request that each such certification
organization not further use or disclose Submissions
for any purpose other than auditing this Web
site for compliance with its certification qualifications.
Of course, we cannot guarantee their compliance
with our requests.
- By using this Web site and by agreeing to
its Terms and Conditions, you grant DRHS and
all other persons or entities involved in the
operation of this Web site the right to receive,
transmit, monitor, retrieve, store and use your
Submissions, including information which may
be privileged and confidential under applicable
state and federal laws, in connection with the
operation of our Web site.

of Certain Information |
- Our goal is to eliminate the possibility of
third party access to information maintained
by the User in the course of providing health
services to a patient concerning the patient
and services provided and your Submissions.
The foregoing is stored with that in mind. Our
staff must abide by our Privacy Statement and
those who violate it are subject to disciplinary
action, up to and including termination of employment.
We will store all information and data in our
database and/or on the database of the host
of the DRHS Web site.
- All clinical data exchanged through the DRHS
Web site is stored on and transmitted using servers
that meet all current security requirements
and regulations.
- DRHS provides a means for its Users to communicate
electronically with other DRHS Users via a 128
bit encrypted internal e-mail system ("Secure
Transmission" or "Intranet").
Therefore, all Secure Transmissions, including
attached files are handled in accordance with
this Privacy Statement. However, if a DRHS User
sends an electronic communication from the DRHS
system to a non-DRHS recipient, or an electronic
communication to a DRHS recipient other than
over the Intranet, then any such transmission
is not a Secure Transmission and is not covered
under this Privacy Statement and may, in some
circumstances, constitute a violation of this
Web site's Terms of Use. If you have a question

- This Web site is not intended to be used by

organizations and individuals, including Users,
located outside of the United States, including
the European Union. User may not and agrees
